Nolan McIntosh

Nolan McIntosh – Midland Director

Nolan McIntosh, DPT, ATC graduated from Lake Superior State University in 2002 with a Bachelor of Science in Exercise Science with concentration in Sports Medicine and an Associates in Health and Fitness Specialist. Prior to continuing his education at Andrews University where he received a Doctorate in Physical Therapy in 2007, Nolan was an Adjunct Professor at Delta College, Southwestern Michigan College, and taught Athletic Training at Michigan State University. Nolan is currently a member of the American Physical Therapy Association and National Athletic Trainers Association.

Nolan first started practicing in the Lansing area and now has been in the Mid-Michigan area for several years. He is the Director of Evergreen Physical Therapy in Midland. Nolan joined Evergreen in order to practice physical therapy in an environment that embodies his philosophy of treatment. His professional interests include orthopedics, sports medicine, manual therapy, and sport specific rehab for local university/high school teams and clubs.

In his spare time, Nolan enjoys staying healthy, being outdoors and spending time with his family. He and his wife, Mindy, with three daughters Haylee, Ava, and Naomi reside in Freeland.

Chad Neal

Chad Neal, DPT

Chad Neal, DPT, is a graduate of Central Michigan University where he completed his undergraduate studies in 2013 with a Bachelor of Science in Health Fitness and Exercise Science. He continued his studies at CMU completing the Doctor of Physical Therapy Program in 2016.

Chad is a current member of the Michigan Physical Therapy Association and American Physical Therapy Association and promotes continuing education and evidence based practice. His clinical experiences include outpatient orthopedics, inpatient and outpatient neuro rehab, and sports medicine. After completing a clinical rotation with Evergreen Physical Therapy in Midland, Chad chose to establish his physical therapy career with Evergreen PT to emphasize his interest in outpatient orthopedics, manual therapy and sports medicine. Chad has recently completed continuing education courses in the functional assessment and treatment of the hip and shoulder through the Gary Gray Institute.

Chad grew up in Auburn, MI and attended Bay City Westem High School where he met his wife Brookelynn. The two recently moved to Kawkawlin, MI after being married in August of 2016.


We are located in the plaza between Entre Amigos and the Grand Traverse Pie Company.
