Carissa Jenema, PT, DPT

Carissa Jenema is a compassionate and well-rounded physical therapist with a deep commitment to delivering personalized, evidence-based care. She enjoys working with individuals of all ages and abilities, drawing on her strong foundation in orthopedic and neurological rehabilitation. Carissa also has advanced training in pelvic health and brings valuable insight into how pelvic floor function can influence a wide range of conditions, even as she continues to expand that area of her practice.
Carissa earned her Doctorate of Physical Therapy from Grand Valley State University (GVSU), where she was actively involved in public health research and received recognition through prestigious awards for academic and professional excellence. Her experience at GVSU cultivated both her clinical skill and her passion for whole-person care.

Athletics has always been a meaningful part of Carissa’s life. In high school, she competed in soccer, volleyball, and cross country, while also helping coach gymnastics for kids. During college, she continued her love of movement by playing on several intramural volleyball teams. These experiences not only deepened her appreciation for physical performance and injury prevention but also continue to shape her patient-centered approach to care.
In her free time, Carissa can be found enjoying the outdoors—whether it’s backpacking, paddle boarding, or hiking with her husband. She brings the same and driven spirit to her work with patients, aiming to help each individual achieve their fullest potential.

Cassandra Davis, DPT, ATC – Physical Therapist and Athletic Trainer

Cassandra Davis brings a unique and well-rounded background to the Evergreen team, having earned both a Bachelor’s degree in Athletic Training and a Doctorate in Physical Therapy from Central Michigan University (CMU). Her dual credentials provide her with an exceptional foundation in sports medicine and orthopedic care, equipping her to treat a wide range of conditions across all age groups.

Originally from the Big Rapids area, Cassandra is proud to serve the community she’s called home. An avid athlete growing up, she played softball, basketball, cross country, and volleyball in high school, which gave her a firsthand appreciation for the importance of staying active and injury-free. Her passion for physical therapy was sparked at a young age, inspired by her own patient experience at Evergreen. That early connection has fueled her dedication to helping others return to the activities they love.

Known for her warm demeanor and eagerness to learn, Cassandra is constantly expanding her clinical knowledge. Outside of the clinic, she enjoys reading, picking up new hobbies, and spending time with her husband and their energetic puppy, Beesly.

George Moore, PTA

George Moore serves as the Therapy Director and has been dedicated to the Big Rapids community for more than 20 years, with a strong focus on orthopedics and sports medicine. A licensed Physical Therapist Assistant and graduate of Baker College, George brings extensive expertise in myofascial release, blood flow restriction training (BFRT), manual techniques for shoulder injuries, and evidence-based approaches for treating neck and back pain.

Known for his warm, caring nature, sense of humor, and compassionate approach, George creates a positive and uplifting atmosphere wherever he goes. He takes pride in treating patients of all ages, helping them restore mobility, reduce pain, and improve their quality of life. His dedication to helping others achieve their best recovery outcomes has made him a trusted and valued professional in the community.
Outside of his professional role, George resides in Canadian Lakes and enjoys traveling with his wife and two daughters, cheering on his daughters at their sporting events, and spending relaxing summer days on the water with his family.

Jennifer Kohn, DPT graduated from FSU in 2008 with her Bachelor of Science degree. She graduated from CMU in 2014 with her Doctor of Physical Therapy degree. She was previously a college and professional athlete and enjoys treating athletes of all levels. Jennifer has recently completed a certification course in Dry Needling. Trigger Point Dry Needling can treat Tendonitis/Tendinopathy, IT Band Syndrome, Plantar Fasciitis, Neck Pain, Headaches, Carpal Tunnel Syndrome, Sciatica, Muscle Strains and more Jen resides in Rockford with her husband, and two children. She enjoys playing soccer, running and bicycling.

Jess Vantroostenberghe, MAT, LAT, ATC, CKTP – Athletic Trainer

Jess Vantroostenberghe is a dedicated and compassionate athletic trainer who brings both expertise and heart to her work with the athletes of Big Rapids High School—on and off the field. She earned her Bachelor of Arts degree in Exercise Science: Athletic Training from Adrian College, followed by her Master of Athletic Training from the University of Findlay in Ohio.

A former high school and college softball player, Jess pitched and served as a designated hitter when not in the circle. Her athletic background fuels her passion for helping athletes perform at their best and recover safely from injuries. She has developed a highly successful concussion management program that streamlines recovery, incorporating vestibular rehabilitation, headache and vertigo care, BPPV management, and a comprehensive Concussion Return to Play Protocol.
Jess resides in the Big Rapids area and enjoys traveling, mountain biking, snowmobiling, hiking, and camping. She values time with her family and friends and embraces outdoor recreation in every season Michigan has to offer.
